Responsibilities

Job Responsibilities and Duties

Essential Job Functions: 

Direct Patient Care (80%):

  • Providing case management for patients
  • Providing specific assessments according to protocols and procedures, clinical competence in procedural skills, treatments, and patient/caregiver education related to wound care management.
  • Acting as patient advocate in delivery and coordination of patient care
  • May float to Medical-Surgical unit

Planning and Organizing (10%):

  • Providing efficient and effective care with the guidance of the physician during clinic sessions
  • Attending the clinics at which you are scheduled and being dependable.
  • Managing multiple patients at the same time with high energy and organization
  • Managing billing and oversight of follow-up with patients

Collaboration / Communication / Follow-up (10%):

  • Collaborating with other health care providers (physicians, Patient Access Director, agencies and Medical Director), regarding clinic and patient needs
  • Monitoring the Cases of patients for whom the Nurse is responsible

Attendance requirements are based on your current role and status of position (FT, PT or PRN).  Please direct attendance requirements to your immediate supervisor and/or refer to the Employee Handbook for more attendance/scheduling details/policies

Qualifications

Education and Experience: 

  • Graduate of an accredited school of Professional Nursing, required
  • Current registration and licensure with the Board of Nurse Examiners for the state or a Compact state
  • Wound Care Certification, preferred
  • Minimum of 2 years medical surgical nursing experience
  • Minimum of 1 year wound care experience (hospital; LTAC; Outpatient; home health)
  • Current American Heart Association BLS & ACLS required
