The social worker reports to the Director of Case Management and is responsible for ensuring that the medically related emotional and social needs of the patients are met and maintained on an individual basis in accordance with policies and procedures, and current federal, state and local standards, guidelines, and regulations. The social worker is an integral part of the team in ensuring the patient’s transition to the next level of care or back to the community is seamless and proactively manages the reduction of readmissions. This position will participate in call rotation with Case Management.
