Responsibilities

The social worker reports to the Director of Case Management and is responsible for ensuring that the medically related emotional and social needs of the patients are met and maintained on an individual basis in accordance with policies and procedures, and current federal, state and local standards, guidelines, and regulations.  The social worker is an integral part of the team in ensuring the patient’s transition to the next level of care or back to the community is seamless and proactively manages the reduction of readmissions.  This position will participate in call rotation with Case Management.

 

Qualifications

Education and Experience

  • Bachelor’s degree in social work from an accredited school of social work.
  • Licensed by the State Board of Social Work.
  • Two (2) years of recent social work experience in a hospital setting, working directly with patients and families.
  • Experience working as part of the case management team preferred.
  • Must be able to function independently and have flexibility, personal integrity and the ability to work effectively with patients, physicians, and hospital staff and support agencies.
  • Must be able to relate to and work with ill, disabled, elderly, emotionally upset and at times hostile people within the hospital.
  • Must possess the ability to make independent decisions when circumstances warrant such action.
